IHTSDO NurSIG: ANI: HIMSS Board of Directors has approved the HIMSS Position Statement on Transforming Nursing Practice through Technology Informatics.Greetings ANI Community members, theI am pleased to announce that the HIMSS Board of Directors has approved the HIMSS Position Statement on Transforming Nursing Practice through Technology Informatics.( HIMSS is also an international organziation and a member of the ANI.) As you all know, nurses are key leaders in developing the infrastructure for effective and efficient health information technology that transforms the delivery of care. The landmark report from the Institute of Medicine& Robert Wood Johnson Foundation, Future of Nursing: Leading Change, Advancing Health Report, determines criteria to transform the nursing profession, leading to new roles and leadership positions for nurses in the redesign of the health care system (this report may be found at http://thefutureofnursing.org/IOM-Report, Though USA-centric, the report may be of interest to all countries as many of the issues are the same). The HIMSS Board of Directors, effective June 17, 2011, has approved the position statement that describes how we must transform nursing practice through technology and informatics. The position statement was developed by leaders from the HIMSS Nursing Informatics Committee, which now represents over 2,900 members, and is intended to not only serve the nursing profession but also the broader healthcare industry and HIMSS membership at large. We are proposing some additions to the current goal achievement findings. Re: IHTSDO NurSIG: Goals for care planningThank you Elaine As we have discussed since kindly posting this, "variance" may raise as many issues as it potentially solves, but is likely to be a legitimate addition particularly for those using SNOMED CT in care pathways rather than more traditional care planning. One thing is that variance is a super-type of both not progressing towards goal and the “divergence from goal”. Potentially it could even include the progressing towards goal in some circumstances as variance is often measured against time, whereas many traditional goals are often simply an identified end-point. The wording did not seem to match up with a goal status in itself. This is certainly an area that would benefit from consideration as we move to a more outcome measure focused agenda in terms of healthcare delivery. The IHTSDO has a specific voice for Nursing in the Nursing Special Interest Group, however there are other much smaller professional groups in healthcare that are not represented formally within the IHTSDO. Re: IHTSDO NurSIG: Response from ZacFollowing SIG discussion in Feb, I provided the feedback below and the attached annotated document to the consultant terminologist working on this topic. Thanks for all contributions. 1. We would suggest that there is a clear statement of the information requirement that this content is supporting so that we can all assess whether the ‘limitation of scope’ is a real problem or the content just looks limited. That would also help to support advice on how to meet the information need using a good information model rather than further precoordination. 2. The three hierarchies listed in section 3.4 are all problematic and it is not clear that any of them are needed. So we can’t support the proposal that a new model should support modelling the three patterns. Since the content mostly came from the UK it would be appropriate to ask Jeremy Rogers to run a query on how frequently these codes are used in UK systems – to begin to understand the impact of any proposed changes. 3. If any of the content is required, the concept names need to be reviewed for international use: district nurse etc may be specific to one country. IHTSDO NurSIG: Content from nursing terminologiesContent Committee project topic: NIC, NOC, NANDA (1109 concepts) (plus OMAHA, PNDS, SABA - 2013 concepts) nursing terminology content already exists in SNOMED CT. Issues related to the context-specific nature of this content (which may not be clearly specified in their FSNs), need addressing . Prioritization needs to be assigned to whether the IHTSDO will take the initiative with respect to implementing updates to content for these terminologies. In 2010 updates to NANDA and PNDS are anticipated. Following discussion at the nursing SIG today, I suggest that we include this topic in issues list. I had promised to review the ICNP-SNOMED-mapping but did not work in AUG (due to stress weeks (suspect mammography)) and found out that the site was already closed when I would restart (Nick Hardiker confirmed this to me) ... IHTSDO NurSIG: Regimes and therapies cleanup - project 115Dear all I was asked to some initial analysis of problems associated with inconsistencies and other quality issues with the (regime/therapy) sub-hierarchy. The attached paper is the initial findings. The suggested changes appear to be straightforward to achieve, have no identified negative impact and would improve the consistency of this area of the terminology, which would reflect the multidisciplinary way in which healthcare is moving where activities can no longer be associated with specific roles or clinical groups. I see no reason this could not be achieved in a single release cycle and the positive benefits could be significant, particularly in defining more authoritative guidance for the planning and management of care using electronic care planning applications and SNOMED CT. I like the options work and the proposal made here, and would welcome the opportunity to explore the options presented in greater detail over coming months, in particular in the light of the attempt to coordinate the design decision here (regime v. therapy) with similar isssues around regime vs. plans & therapies vs. treatments, along with a desire to characterise the relationship between SNOMED CT content and the developing 13940 suite of ISO standards. The first workshop in the series was led by William Goossen who provided an overview of HL7 standards and the implications for nursing as a stimulus for discussion. The second workshop was led by Anne Casey who provided an overview of two important ISO standards for nursing terminologies and for continuity of care. This third workshop will be led by Peggy White and will provide an overview of C-HOBIC (Canadian Health Outcomes for Better Information and Care). The SNOMED CT® encoded Nursing Problem List Subset, intended for use in patients’ problem lists, is an output of the Unified Medical Language System® (UMLS®) Metathesaurus® that is based on nursing diagnosis concepts found within the Metathesaurus. Purpose and use of subset The main purpose of the Nursing Problem List Subset of SNOMED CT® is to facilitate the use of SNOMED CT® as the primary coding terminology for nursing problems used in care planning, problem lists or other summary level clinical documentation. The use of a common list of SNOMED CT® concepts will maximize data interoperability among institutions. Local problem list vocabularies often need to expand to satisfy specific user needs. Institutions that are using their own nursing problem list vocabularies are encouraged to map them to SNOMED CT® with a focus on these nursing diagnosis concepts to facilitate data interoperability. The UMLS Terminology Services (UTS) includes a SNOMED CT browser that may be used for this purpose. The SNOMED CT® Browser is available through the SNOMED CT® menu of the UMLS Terminology Services. You must have a UMLS license to access the list. To obtain a license, free of charge, go to http://www.nlm.nih.gov/databases/umls.html. Additional resources from the UMLS For each SNOMED CT® concept in the subset, the corresponding UMLS CUI (concept unique identifier) is listed. By using these identifiers, users can access resources available in the UMLS e.g., additional synonyms, text definitions for many terms, corresponding codes in other terminologies, etc. The file identifies what we hope is a useful Nursing Problem List Subset of SNOMED CT®, but does not include all the information likely to be required to make effective use of SNOMED CT® in an application, such as synonyms that may include more clinician-friendly terms than the SNOMED CT® fully-specified name. The identifiers in the file can be used to extract more complete information for these concepts from either the UMLS release files or the SNOMED CT® native format files. I am soliciting comments from you because you are a member of the SIG. The deadline for comment is Friday. If you could review and get comments back to me on the attached document by end-of-day Wednesday, I would really appreciate it. Thanks...Susan As part of the Terminology Summit transition, the IMIA NI Healthcare Standards Working Group is hosting a series of on-line workshops to further the valuable work of the Summit in providing strategic direction to nursing informatics, with a particular focus on supporting the management of nursing information. The first workshop in the series was led by William Goossen who provided an overview of HL7 standards and the implications for nursing as a stimulus for discussion. The second workshop was led by Anne Casey who provided an overview of two important ISO standards for nursing terminologies and for continuity of care. The third workshop was led by Peggy White provided an overview of C-HOBIC (Canadian Health Outcomes for Better Information and Care). We are extremely fortunate to have William Goossen back for our fourth webinar. Dr. Goossen will provide an overview of Detailed Clinical Models. Detailed Clinical Models provide a way of organising health information, combining knowledge, specifications for data elements, relationships between data elements, and terminology, into reusable information models. There are no right or wrong answers. We are interested in your perspectives given your knowledge of nursing and informatics, and what is likely to happen in health care in the future. The results of this survey will be used to refine priority statements, which will then form the basis of another questionnaire. This second questionnaire will be distributed to individuals who volunteer to complete it. Through this process we hope to gain some consensus from the International Nursing Informatics community on the key research priority areas for the next 5-10 years.
