A colleague who is working on the ICD-11 mapping project came across the concept 70388009 |Postoperative cataract syndrome (disorder)|. This is not a term listed in either ICD-11 or ICD-10.

A Google Scholar search for the term returned just a single reference which is from an Royal College of Ophthalmologists audit from 2020. The term is mentioned once, where it is included in a list of diagnoses, but is not expanded on in any meaningful way. 
They also searched using the synonyms, and both 'vitreous tug syndrome’ and 'Irvine-Gass syndrome' (IGS) return more results, including a few papers suggesting that another term for these conditions is 'pseudophakic cystoid macular edema (PCME)'. This exact term does not appear in either SNOMED CT or ICD-11; however, it would probably be synonymous with 1217634001 |Cystoid macular edema following cataract surgery (disorder)|, which has an equivalent ICD-11 foundation entity here - http://id.who.int/icd/entity/1314096397

There are also terms for posterior capsular opacification in SNOMED which seem to be referenced as postoperative complications:



Would you be able to advise if 70388009 |Postoperative cataract syndrome (disorder)| is still a valid concept and distinct from other existing concepts mentioned above or is it a duplicate of one of these?

  1. I've never heard of it!  And can't find anything meaningful when searching.  The issue may then be what do we collapse it under when retiring?  I think given it is so vague, perhaps we do not map it to anything?

    Based on the synonyms listed, it should perhaps be linked to one or more of those concepts.

  3. Thanks Michael.  Agree then, it can be collapsed under one of the others.  Elaine - is there a way to see which codes are most commonly used in publicly accessible datasets, for example (probably not)?  If not, "pseudophakic cystoid macular edema" would get my vote (noting the US spelling).

  4. Thank you both.  We do have some NHS primary care data (so not specialist centres/hospitals) with the following stats:

    • 193387007 |Cystoid macular edema (disorder)| used 14,300 times.
    • 312920004 |Postoperative cystoid macular edema (disorder)| used 340 times
    • 70388009 |Postoperative cataract syndrome (disorder)| zero use

    Are there any others you would like me to check?

  5. Thanks Elaine.  I think 312920004 is the one to go for, which should be a subtype of 193387007, which is more general.
