Discussion surrounding use of Inheres in used twice...extending definition of inheres in?  Length does not "inhere in" the neoplasm nor the margin, strictly speaking.

Create high level, intermediate primitive concept for distance between margin and neoplasm.  Current classification forces distance to be a subtype of neoplasm observable...shorter term fix until modeling
