  2. Nicola Ingram  I updated some ranges for the model.  There are over 80 concepts are modelled by the due to relationships. It might be better to restrict this template to the simple pattern by removing the due to attribute. Otherwise, you will need to address all association patterns. 

  3. Nicola Ingram , I have updated the cardinality for |finding site| and range for |occurrence|. The misalignment report does not include complex associations. I think this template is ready for implementation unless you need to make further changes. Cheers,  Yong

  6. It is causing a problem for the group remodel process to have both a grouped and an ungrouped pathological process - we've not seen that before.   I could add some code to be more strict with the alignment of ungrouped, optional attributes.   

    The template does at least explain why I've been seeing odd results in QI-297.   The remodel is attempting to fill the template as much as it can, but in the this case, it should give up sooner since the ungrouped attribute is optional.

Hi Nicola Ingram  the additional Causative agent = <<  30621004 |Autoantibody (substance)| and Pathological process = <<  472963003 |Hypersensitivity process (qualifier value)| seems to be used for representing type II hypersensitivity conditions, such as Myasthenia gravis, Graves disease. I think these two attribute/value pairs need to be grouped together. It might be useful to create a new template for type II hypersensitivity conditions. Then, autoimmune disorder would be a generic template without these two attribute/value pairs. Please check with Bruce Goldberg  for his expertise in immunology. 


    1. Type II hypersensitivity may be autoimmune or non-autoimmune (e.g. transfusion reactions) so for autoimmune type II hypersensitivity would need 2 role groups one containing a pathological process of autoimmune and the other a new child of hypersensitivity process, type II hypersensitivity process. Also type II hypersensitivity is comprised of 2 subtypes IIA (antibody mediated cellular damage) and IIB (formerly type V, cellular dysfunction due to receptor stimulation as in Graves disease or inhibition as in Myasthemia gravis)

      We might need to consider at some point creating templates for type III and IV hypersensitivity as well. The template for allergy covers type I hypersensitivity (422076005 |Immunoglobulin E-mediated allergic disorder (disorder)|

  8. Thanks Bruce Goldberg I think we should produce a separate template for hypersensitivity conditions that are autoimmune. I am wondering if the types of hypersensitivity can help to identify hypersensitivity conditions that are autoimmune. The guidance would be helpful for authors to choose the right template for modeling. Then, this template would only represent specific autoimmune conditions (exclude hypersensitivities). The other issue with the current template is that it won't facilitate identifying the misaligned concepts since these additional attributes are optional. 

    1. Yongsheng Gao I would create a new template for hypersensitivity disorder after adding new qualifier values under hypersensitivity process for type II, IIA, IIB, III and IV hypersensitivity processes. As alluded to above, 769260004 |Immunoglobulin E-mediated allergic process (qualifier value)| is the same as type I hypersensitivity process. A second optional role group could be added with a pathological process of autoimmune and a finding site to define those disorders that represent one of the hypersensitivity processes and are autoimmune.

  9. Hi Bruce Goldberg , I have updated the template language to align with the model with further changes. The attribute |Occurrance| is included in the role group because it is normally grouped with other attributes. The attribute |Causative agent| is included in the model because it has been used for modelling existing concepts. It looks like that 370135005 |Pathological process (attribute)| = 263680009 |Autoimmune process (qualifier value)| is not part of the role group1. Should |Pathological process| should be part of the role group 1 or a different pathological process other than Autoimmune process could be part of role group 1?

  10. Hi Yongsheng Gao. Organism should not be a value of causative agent for autoimmune diseases as the organism itself does not cause the autoimmunity but rather the immune response to the organism. In those cases due to or due to and after relationships to the infection caused by the organism can be used. An additional pathological process can be included in role group 1 in order to define those autoimmune disorders that represent hypersensitivity once these more specific hypersensitivity processes are created. I originally had this in the template.

    1. Hi Bruce Goldberg, I have removed the organism from the template language. Do you mean that there will be two separate pathologic processes? Then, this template will need to have two role groups. 

  11. Hi Yongsheng Gao. Yes, this will need 2 pathological processes if we want to describe specific hypersensitivity processes such as delayed type hypersensitivity as these are not necessarily autoimmune. Same thing for Allergic contact dermatitis. If we want to add delayed type hypersensitivity as a pathological process we would need it in a second role group from allergic contact hypersensitivity because delayed type hypersensitivity is involved in diseases not considered to be allergic. I will modify the template for now but I will need to do more testing. Modeling autoimmune disorders will be difficult as the mechanisms will need to be researched and in some cases more than one mechanism may be involved and in many the mechanisms may be unknown.
