Realize that this is a work in progress, but I do not understand what the goal is. The attributes listed are a combination of those allowed for situation and procedure, yet the title of the page is "History of finding/disorder". The rules for description generation also relate to procedure and not situations. If I am missing something, please educate me.

I'm not sure how to make this terming generation correct, and whether this template should be for all findings/disorders or just with disorders.

Done is not a valid value for finding context. This would be things like "present", "Absent", "Unknown". I am not clear from your question what the specific difficulties are, but then I am not working on templates.
