Also a CRS request received for a new concept - Opioid use disorder (disorder) - reference given in DSM-5.  ICD-11 has Disorder due to use of opioids, Opioid use disorder and Disorders due to abuse of opioids as synonymous https://icd.who.int/dev11/f/en#/http%3a%2f%2fid.who.int%2ficd%2fentity%2f111507461 This would seem to indicate the requested new concept could be a synonym on 5602001 |Opioid abuse (disorder)|.

Are we able to move this discussion forward or do we still need input from substance use experts?

Minute: It was agreed Piper would pull together the main conceptualisations relating to this issue and post to share with the group.  Scott will talk to some colleagues who are experts in this field as it is a difficult issue to solve.

191736004 |Obsessive-compulsive disorder (disorder)| is currently a subtype of 197480006 |Anxiety disorder (disorder)| is this correct as in ICD-11 they are siblings.  Is anxiety always a feature of OCD?

Minute:  Following discussion it was agreed that although this often involved anxiety it does not always so it would be consistent with both ICD-11 and DSMV to move this from 197480006 |Anxiety disorder (disorder)| to 74732009 |Mental disorder (disorder)|

3111479008 |Organic mental disorder (disorder)|

Colleagues are reviewing content relating to intoxication delirium under 191492000 |Delirium caused by drug (disorder)| e.g. 18653004 |Alcohol intoxication delirium (disorder)| and 8635005 |Alcohol withdrawal delirium (disorder)| which has raised some questions that need clinical input.

Delirium in SNOMED is classified as a descendant of 111479008 |Organic mental disorder (disorder)| which in turn is a subtype of 74732009 |Mental disorder (disorder)| and 81308009 |Disorder of brain (disorder)|.

Descendants of Organic Mental Disorder include:
52448006 |Dementia (disorder)|
2776000 |Delirium (disorder)|
286933003 |Confusional state (disorder)|

Looking at ICD-11 and DSM-V delirium and dementia are now subtypes of neurocognitive disorders.

Questions:

  1. Is the current concept Organic mental disorder still clinically useful?
  2. Should the delirium and dementia concepts be moved to be subtypes 709073001 |Neurocognitive disorder (disorder)| ?
