The New Feedback modal is presented. It contains the following: 

Name of the reference set or member 

Status: Open or closed 

Username and avatar 

Feedback Topic field (required) 

Mark Feedback Private checkbox (optional)

Feedback text box (required)

Start Discussion button: The button is unavailable until all required fields are filled in (feedback topic and text). 

Enter a title or description in the Feedback Topic field. 

Select the Mark Feedback Private checkbox, if desired. 

Enter the feedback in the Feedback text box. 

Press the Start Discussion button. 

NOTE: Discussions marked as private may only be seen by the author of the reference set, the author of the discussion, and admin.
